Here you will find all the documents and forms needed to register for our programs. Please fill out, sign and upload using the button below.
Completed documents can also be emailed to admin@countryclassroom.org, or mailed to 2136 Huntersland Rd., Middleburgh NY, 12122. Medical documents can be faxed to us at 518-707-1433.
General Enrollment
To complete your initial enrollment, please submit the following:

Tuition deposit of $500 per child - Tuition deposit payments should be mailed to 2136 Huntersland Rd., Middleburgh NY, 12122. Your registration is not complete and your spot is not saved until we receive your deposit payment.

Completed Registration Form

NYS Health Exam Form - Medical documents can be faxed to us at 518-707-1433.

Permission to Administer Medications - To be filled out by your child's physician and you, only if your child needs medication administered or available during school hours.

Current Immunization Record - signed by a physician or a school health record. Please note that your child needs to be up to date (or in progress) on their required immunizations to attend any schools in New York State. Medical documents can be faxed to us at 518-707-1433.

Family Handbook Acknowledgement Form - please read through our Family Handbook, contact us with any questions, and then sign and return the Acknowledgement form.

Records Release Form - if your child is transferring from another school, sign and return to the school and we will forward it on to your child’s previous school.
Summer Camp
Fill out our online registration form.
Payments should be mailed to 2136 Huntersland Rd., Middleburgh NY, 12122, or pay online here. Your registration is not complete and your spot is not saved until we receive your full payment.
You will also need to download and complete or obtain the following:
